If your doctor has said that you may order a repeat prescription of your medication without a review, please use the right hand side of you prescription form to do this.
It will be necessary for you to see your GP regularly to review any medication which you are taking. The date for this review is printed on the right hand side of the form.
If this date is due, please arrange a routine appointment with your doctor in plenty of time for your prescription to be issued and dispensed.
If you have regular blood tests in connection with your medication then please arrange an appointment for these in advance of your review.
When requesting repeat prescriptions please allow at least two working days from the time you request the prescription to when you collect it.
If you would prefer to collect the prescription directly from the local pharmacy please mark the request form accordingly.
You may feel you would benefit from pre-paying for your prescriptions or you may be exempt from costs.
If you are not sure if you qualify for exemption — please ask at the reception desk.
