Registering with the Practice
If you live within the practice boundary and wish to register with us, please complete a registration form. This provides us with the necessary details to care for you until your medical records are transferred from your previous GP.
You can:
-
Print the form below and hand it in at Reception, OR
-
Register online by clicking here; Register with Sandwich Medical Practice
Important Information
-
Everyone is welcome in general practice - you do not need proof of ID, address, immigration status, or an NHS number to register or see a GP.
-
Registrations are made with the Practice as a whole, not with individual GPs. However, you may express a preference for a particular GP. This will be noted on your records, and we will do our best to accommodate your request.
-
If you change your address within the practice boundary, your telephone number, or any other personal details, please notify the Practice as soon as possible.
-
The Practice does not accept out of area registrations.
Please note if you move outside of the practice boundary you will need to register with a new GP practice closer to your new address. You can find your nearest GP practice here; Find a GP - NHS
Our Practice Boundary
The map below shows our practice boundary. If you are unsure, enter your postcode here (Find a GP - NHS) to check whether you are within the area and eligible to register with us.

Temporary Patient Registrations
If you are staying in the area temporarily, you can register with us as a temporary patient. This allows you to receive care from our practice while remaining registered with your permanent GP. If you wish to register as a temporary patient, please complete a GMS3 Temporary Registration Form available at Reception.
Key Points
-
Temporary registration is available for up to 90 days
-
After 90 days, you can either re-apply as a temporary patient or register permanently if you are staying longer
-
You cannot register temporarily at a practice in the area where you are already permanently registered.
- Practices are not obliged to accept temporary patients but must provide emergency care.
-
This option may be suitable if you are living away from home for work, study, or travel or visiting the UK from abroad and need medical care.
Accessibility and Disabled Facilities
We want everyone to feel welcome and comfortable when visiting our practice - the building is fully accessible, with ramps for easy entry and a disabled toilet on site. There are wheelchairs available in the foyer if required, and disabled parking spaces are provided in the Guildhall Car Park, managed by Dover District Council, just outside the practice.
Non-English Speakers
These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.
Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups.
